Rating criteria
| Rating | Criteria |
|---|---|
| 20% | Limited at shoulder level (minor); 30% at shoulder level (major). |
| 30% | Midway between side and shoulder (minor); 40% midway (major). |
| 40% | To 25 degrees from side. |
